Interstate 5 to the Mile Of Cars Way Exit, head west (towards the bay), Cleveland Ave is the first Right you can make. Free Form is located 3 blocks on the left at 1912 Cleveland Ave. (For those of you who wish to avoid traffic it can get bad (sometimes really bad) after 3pm.)
